  • CGCC Name Update Form

    Students can update their name in CGCC's information system to reflect their preferred name or to officially change their legal name.
  • Instructions

    • Complete this form
    • Click 'submit'
    • Upon submission, Student Services will send a confirmation asking you to verify your preferred name. You must respond to the confirmation for your name to be changed.
    • If changing your legal name (first and/or last), provide the required documents to Student Services.
  • Personal Information

  • If you are changing your legal last name, you'll need to provide Student Services with one of the following official documents:

    1. Signed Social Security card displaying the new name or
    2. Official Name Change Document, such as a:
      • Divorce decree
      • Marriage license
      • Immigration document
      • Court order for a name change
  • Verify and Submit
